New Patient Form Please complete this form prior to your first visit if you are a new patient to our practice.

Appointments
Your scheduled appointment time has been reserved exclusively for you. If you are unable to keep your appointment we kindly ask that you give a 48 hour notice. This courtesy, on your part, will make it possible to give your appointment to another patient.

Insurance
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full. If you have any questions please call our front office staff and they will be happy to assist you.

Emergencies
Our office provides on-call service for emergencies 24 hours a day, 7 days a week. After hours call

Online Patient Bill Pay
Babineau Dentistry is pleased to offer its patients the ability to pay their bills online by using a secure payment portal. Payments are normally posted to your account within 36 business hours. Credit card information is used only for payment processing and fraud prevention. Credit card information and other sensitive personal information required to process a payment is not used for any other purpose by our office and will not be kept longer than necessary for providing these services.

Financing
Our office will be able to assist you with a financing program for elective procedures. Patients may be pre-approved for financing with a low interest monthly payment plan. Our office manager would be pleased to provide you with further details.

Billing and Payment
Co-payments, co-insurance and deductibles are due at time of visit. We accept all major credit cards (Visa, MasterCard, Discover, AMEX), cash, check, money order or cashiers checks.
